Mr. Diabetes® Achievement Award 

Contest Rules and Eligibility Requirements

 

1. Eligibility: The Mr. Diabetes® Achievement Award is open only to legal residents of the United States 18 years of age of older, or if a minor, with consent of legal guardian(s) who:

a. Have Type 1 or Type 2 Diabetes or diagnosed Pre-Diabetes.

b. Have taken steps to implement the Defeat Diabetes® Motto: Awareness + Action = Prevention.

c. Have dedicated time to diabetes prevention and/or awareness outreach campaign.

Limit: One entry per eligible individual.  An eligible individual is a person who meets all eligibility requirements.  Employees and their immediate families (spouse, parents, children, siblings) and their household members (whether or not related) of Defeat Diabetes Foundation, it’s advertising, promotional agencies and web masters/suppliers and their respective parents, subsidiaries, and affiliated entities may not enter. Void where prohibited.

2. How to Enter: Entry Forms can be obtained on the Internet at www.DefeatDiabetes.org (click on "How to Apply" at the bottom of this page). When accessed, print the Official Entry Form. The Official Rules and Entry Form will be available on the website as of November 24, 2003. Or send a SASE to the Defeat Diabetes Foundation Achievement AwardsAdministrator at: 501 150th Avenue North, Madeira Beach, FL, 33708 for official contest rules and application.

Entries may be submitted by (a) individuals meeting the eligibility criteria, or (b) teams consisting of at least one individual who meets the eligibility criteria (c) if entries are submitted on behalf of a team, each member of the team must sign the entry form, which will signify that they consent to entry in the contest.

Each entry must include a 500 to 1,000-word summary (one to two pages, single-spaced, typed) detailing:

1.   What project(s) you have undertaken that have aided in creating awareness about diabetes or preventing diabetes.

2.   Why you think awareness and prevention are important in the effort to fight diabetes.

Entries need not be on the Official Entry Form but must include all of the required information: Name, street address, city, state, zip code, telephone number, e-mail address (if available), date diagnosed with diabetes or pre-diabetes, and three (3) individuals to verify the accomplishment and a signature attesting that you comply with all of the eligibility criteria. All Entry Summaries must be truthful in all respects and be the sole and exclusive work of the entrant. The entrant  warrants and represents that the entry is truthful in all respects and does not infringe on any rights of copyright, trademark, privacy or publicity or any other right of any person, or as applicable, any entity. Any entry that the sponsor deems to be inappropriate, or containing material that is not proper for publication (as solely determined by Sponsor) will be disqualified. Entries not satisfying the Official Rules in all respects will be disqualified.

All entries become the property of Sponsor and will not be acknowledged or returned. All entries must be received by February 29, 2004. Defeat Diabetes Foundation is not responsible for late, illegible, incomplete, stolen, postage due, lost, mutilated, separated, or misdirected entries. Limit one entry per eligible individual. By entering you agree to be bound by these Official Rules and the decisions of the Sponsor, its designee, and/or the judges (see below), which are final and binding in all matters relating to the Contest.

Judging: Judging will be accomplished in several stages. First, representatives of the Sponsor will review the entries to ensure they meet all eligibility criteria and comply with entry requirements. Second, a panel of three judges drawn from the diabetes community, which may include but are not limited to officials of professional associations, healthcare professionals who treat people with diabetes, nutritionists, editors or employees of diabetes publications, and employees of Sponsor.

Ten (10) semi-finalists will be selected during the first round of judging. The entries of the ten (10) semi-finalists will be reviewed by a panel of Judges in the second round which will include Mr. Diabetes® (who will also break any potential tie votes) and will be based on the following criteria:

a.   How did the project impact/inspire a larger audience about the role of prevention and awareness in eliminating the threat of diabetes.

b.   The clarity of the activity and the ability to replicate the project in other communities.

4. Prizes: Grand Prize: $3,500; Second Prize:  $1,500; Third Prize:  $500 will be awarded. Prizewinners will be allowed to utilize the prize for any purpose. At the Sponsor’s sole discretion, the winners’ likeness and name may be used for promotional purposes without any further compensation.  For All Prizes: Taxes on prizes are the sole responsibility of winner(s). If winner cannot accept prize as stated, prize will be forfeited and awarded to an alternate winner. Prize cannot be transferred, assigned, or substituted by winner. If the winner of any prize is a minor his/her prize may be awarded in the name of, or to, minor’s parent/legal guardian who must execute all documents and agree to all obligations and undertakings of the winner, both on behalf of himself/herself and winner stated in these Official Rules, and failure to do so may cause prize to be forfeited and awarded to an alternate winner.

5. Notification of Winner: Semi-finalists will be notified by approximately May 30, 2004. Grand Prize Winner will be notified by approximately June 30, 2004 by phone and/or certified mail.

6. General Conditions: If Winner is found not to be in compliance with these Official Rules or otherwise ineligible, prize will be forfeited and awarded to an alternate winner. In addition, within 20 days of notification, the Grand Prize winner must sign a W-9 for the release of the award. Where permitted by law, winner(s) grants and agree(s) to confirm to allow Sponsor the right to print, publish, broadcast and use, worldwide in any media now known or hereafter developed, including but not limited to the world wide web, and Internet, at any time or times, the winner's name, portrait, picture, voice, likeness, biographical information and winning entry for advertising, trade and promotional purposes without additional consideration and without notice, review or approval. By accepting prize, Winner(s) release, discharge and agree to hold harmless Defeat Diabetes Foundation, advertising and promotion agencies, and all of their respective employees, officers and directors, representatives and agents, from and against any and all claims, losses or damages of any kind, including personal injury, death and or property damage, arising, in whole or in part, directly or indirectly, from winner (s’) acceptance, possession, use or misuse of any prize awarded or participation in this Contest, or participation in any Contest related activity.

7. Limitations of Liability: Sponsor is not responsible for any incorrect or inaccurate information, whether caused by website users, tampering, hacking, or by any of the equipment or programming associated with or utilized in the Contest and assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, technical error, theft or destruction or unauthorized access to the website. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the Contest or website(s), to be acting in violation of the Official Rules; or to be acting in a non-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. Sponsor is not responsible for injury or damage to participants' or to any other person's computer related to or resulting from participating in this Contest or downloading materials from or use of the web site(s). No responsibility is assumed for any computer, telephone, satellite, cable, network, electronic or Internet hardware or software malfunctions, failures, connections, or availability, or garbled, corrupt or jumbled transmissions, service provider/Internet/web site/use net accessibility, availability, or traffic congestion, or any technical error. If, for any reason, the Internet and/or mail components of the Contest are not capable of running as originally planned by reason of infection by computer virus, worms, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes, which, in the sole opinion of Sponsor, corrupt or affect the administration, security, fairness, integrity, or proper implementation of the distribution of entry forms via Internet and/or mail, Sponsor reserves the right to cancel, modify or suspend the distribution of entry forms via Internet and/or mail and select the winners in a manner deemed fair and appropriate by Sponsor.

8. Winners List: A list of Winner(s) will be available at the sponsor’s website at: www.DefeatDiabetes.org after June 30, 2004 or by sending a request for a winner’s list together with a self-addressed stamped envelope to:
