Fetus Size, Not Mother's Blood Sugar, Is The Most Important in Treating Gestational Diabetes

If this method were widely accepted, it could reduce the number of women who require treatment for this disease, which is a form of diabetes that appears during pregnancy and can lead to large birth weight babies. Babies who are born overweight are often more difficult to deliver and are at increased risk for other health problems at birth, as are their mothers.

The study, by researchers in Germany, found that giving insulin only to the mothers of babies whose abdominal size was in the 75th percentile or higher while in utero produced better results than giving it to all mothers whose blood glucose levels were higher than normal. Researchers measured the developing fetus using ultrasound technology.

In fact, the study found, treating some pregnant mothers whose glucose levels were elevated resulted in low birth weight babies, which is also dangerous to the child's health.

"The optimal treatment strategy during pregnancy would therefore be a treatment program to prevent both high and low birth weight neonates," wrote Dr. Lois Jovanovic, of the Sansum Diabetes Research Institute in Santa Barbara, CA, in an accompanying editorial.

Dr. Jovanovic's editorial strongly supports using the ultrasound testing treatment strategy in lieu of maternal glucose monitoring, a reversal of her prior position on the issue. The new method appears to be more effective as well as more cost-efficient, she wrote, in a paper titled, "Never Say Never in Medicine! Confessions of an Old Dog!"
